Player Information Form:
All quota and alternate players must complete the Player Information Form and return it to the District Endorser in order to be considered for selection to the tournament.
Midwest endorders will make selections from all district alternates.
Check w/your district endorse for your district Status: District Quota List# ___ or District ALT List#___ .
Call the tournament for your Sectiona Alternate status.

Alternates:
All Alternates must call (317) 259-5377 or (317) 253-4526 between 5 and 7 p.m. Indy time- same as Chicago on Wednesday 6/23, or on 6/24, 7 a.m. to 7 p.m. for alternate selection status:
1) "IN";
2) "Standby Alternate"("next in") #_?_ ; or
3) Not IN - too low on the standby list to have any real chance of play.
Standby alternates may come to Indy but are not promised a spot in any draw.

ALL district Quota Players and "selected" alternates (MW endorser will select from all district alts.) must register in Indy at The North CentraL HS 1801 E 86th St, Indy 46240 :
Boys and Girls 16 players register between 12:30 p.m. and 2:30 p.m. (Indianapolis time) First Match Schedule Card will be passed out.
Boys and Girls 18 players register between 12:30 and 4:00 (Indianapolis time) First Match Schedule Card will be passed out.
Doubles must be confirmed at Check-In.

Dress Code
All Players must wear appropriate tennis attire!
No shirts or hats with references to drugs, alcohol, tobacco products or sexual or bodily functions.
No cut off or torn shirt sleeves and/or shorts are allowed.
No rolled down waistbands are allowed.
The bottom of the shirt or top must meet the top of the shorts or skirt when arms are at rest throughout the entire match.
Girls - No spaghetti strap tops or tube tops.
Boys - No tank tops.

Order of Play!
Saturday and Sunday - 1 rd of singles and 2 rds of doubles
Monday and Tuesday - 1 rd of Main + 2 rds of Cons + 1 rd of doubles (semi or finals, if on schedule.)
Wednesday - Quarters and Semis of Main + 2 rds of Cons + doubles if necessary.
Thursday - Finals and 3rd place Main draw and Quarters and Semis of Cons.+ doubles if necessary.

District Endorsement Required---FIC may not finish. Eligible
player's names must appear on the official district endorsers list
to be considered for selection to play.
Players are selected from a designated Section or District ranking list.
Entry to this tournament is open to specified residents only. Entries must be submitted by Saturday, June 19, 2004 11:59:00 AM Indiana Time. Only USTA members can register online if available.
