MEDICAL RELEASE FORMS
As with all USTA National events, you must fill out a medical release form and fax it to us before you arrive at sign in. Please click the attached link and fax it to 619-221-9076. THIS FORM MUST BE FAXED BACK TO US BY WEDNESDAY, AUGUST 5TH AT 5PM. IF WE DO NOT HAVE THE FORM BY THAT TIME YOU MAY BE PULLED FROM THE DRAW AND REPLACED BY AN ALTERNATE.

PLAYER PROFILE FORM
Each year we put together a player profile book that we offer to college coaches. This is free service for you and your first opportunity to let coaches know who you are, when you are going to graduate, and what your interests are for your future. Please take the time to complete the online form BEFORE YOU ARRIVE FOR SIGN IN. The deadline for submission of the Player Profile is WEDNESDAY, AUGUST 5TH at 5pm. WE ARE ATTEMPTING TO RE-OPEN THIS SITE. PLEASE CHECK AGAIN ON THURSDAY IF YOU DID NOT GET IT IN.

SIGN-IN
All players must sign in during player check-in from 12-4:30 pm on Saturday, August 8th at the Barnes Tennis Center. If you are going to be late for check-in you must request permission from the tournament director at least 24 hours in advance or you may lose your spot in the draw and an alternate will be put in your place.

DOUBLES SIGN-IN
Doubles sign in will be open until Saturday, August 8th at 4pm. Even if you entered doubles on your original entry form, you need to sign in with the tournament referee or tournament director before that time to confirm your entry. You may email your entry and partner's name to sbickham@ytsd.org as well.

DOUBLES TEAM LIST
If you plan to play doubles please check the competitor list and make sure your name is on it. TDM does not allow for you to check and see if you are with the right partner, so please email to sbickham@ytsd.org that you are partnered up correctly.

PRACTICE COURTS
There will be limited practice courts available at the Barnes Tennis Center until noon on Saturday, Aug. 8th. Call 619-221-9000 to reserve. Courts will be available on a first come first serve basis at the Peninsula Tennis Club, call 619-226-3407 for directions at the University of San Diego (8) and Pt. Loma Nazarene University (6). Maps will be available at check-in and at the tournament hotel.

HITTING LESSONS
We will have pros available for hitting lessons beginning Thursday, Aug. 6th. Please email sbickham@ytsd.org to sign up. Cost is $50/hour.

OPENING CEREMONY, SAT. AUGUST 8TH
Please make a point to come to the Opening Ceremony on Saturday at 5 pm after Sign-in. We will have a full Military Brass Band and Color Guard Presentation followed by the "mandatory" players meeting and a nice pasta dinner from Pat & Oscar's Restaurant.

"FEARLESS TENNIS" EXHIBITION
Complementary Special Event: “Fearless Tennis Exhibition Match with Commentary on the Mental Game” August 8th from 6-7PM after the Opening Ceremonies. Plan to attend and not miss this truly unique event never before offered at a National Championship. The Barnes Tennis Center and Youth Tennis San Diego are pleased to continue its tradition of player development and life preparation by providing a one of a kind experience in tennis.

MATCH SCHEDULE
We will be starting the Main Draw of Singles and Doubles on Sunday, August 9th and continue with one match per day in the main draw thru the following Sunday. Consolation singles will begin on Tuesday and we will have two matches daily in the consolation draw.

DRESS CODE
This is a USTA National Championship Event and with that in mind we ask all participants to wear tennis attire during their matches. T-shirts will be discouraged unless it is the tournament t-shirt that we give you at check-in. Please pack accordingly.

Facilities: All matches will be held at the Barnes Tennis
Center, 4490 W. Point Loma Blvd. From Airport take rt. on Harbor
Dr. to Nimitz to rt. on W. Point Loma Blvd. We are 10 minutes from
airport. The facility has 21 hard courts, 4 clay cts, snack shop,
player shower/lounge, etc. Amenities: All players will receive
T-shirts & gift bags at check-in. Racquet stringing and a

This event is restricted to the following Jr. players: U.S. Citizens; Permanent Resident Aliens; Diplomats, Refugees/Asylees; CTA. Refer to USTA Regulation III.A.2a.

Players are selected from a variety of National Lists and generally require Section endorsement.
Entry to this tournament is restricted. To see how entrants will be selected, follow the “Selection Process” link to the left. It is the player’s responsibility to check the selection list(s) after registering online to be sure that a check mark appears beside the player’s name on the list, in order to insure proper registration. Entries must be submitted by Thursday, July 16, 2009 12 Noon Eastern Time. Only USTA members can register online if available.
