PROOF OF CITIZENSHIP
IF THIS IS YOUR FIRST QUALIFIER, YOU MUST PROVIDE PROOF OF CITIZENSHIP BEFORE THE DEADLINE TO PLAY IN THIS TOURNAMENT! See below for acceptable documents.

PROOF OF CITIZENSHIP OR LEGAL RESIDENCY REQUIRED
District Qualifiers are open to citizens of the U.S., permanent resident aliens in possession of a valid alien registration card, individuals who are applicants for adjustment to lawful permanent resident status and who can present a USCIS receipt for a pending unadjudicated form I-485, aliens who have resided continuously in the U.S. for more than one year and who are members of families of persons in the diplomatic or consular corps and aliens who have been granted Refugee Status, Asylee Status or Temporary Protected Status. Proof of the above must be presented to the district office prior to the entry deadline as follows: 1) U.S. Birth Certificate, 2) U.S. Passport, 3) permanent resident alien card (green card), 4) I-485 letter. EXEMPTIONS
There are no exemptions that will allow for district endorsement. Players who are unable to play due to a bona fide and documented injury and would have been seeded in the event (based on the number of allowable seeds for the size of the draw) may request endorsement on the MSITA Alternate list prior to the tournament entry deadline. The MSITA Junior Competition Committee will make the final decision to honor any such requests.

FORMAT OF PLAY
All matches will be best of 3 tiebreak sets with a 10-Point Match Tiebreak played in lieu of a 3rd set. Matches will be played with green balls on a 78' court.
Players should expect to play up to 3 match each day.

PLAYER CONTACT INFORMATION
The USTA Legal Department has determined that they cannot provide contact information to tournament directors unless the customer provides the contact information during the registration process. If you do not provide an email address and/or telephone number when you register for this tournament then we will be unable to notify you about any possible changes in the tournament.

Players are selected manually by the tournament director after the close of registration.
Entry to this tournament is open to specified residents only. Entries must be submitted by Monday, May 13, 2019 11:59:00 PM Central Time. Only USTA members can register online if available.
