MEDICAL RELEASE FORM
USTA requires a medical release SIGNED BY THE PARENT OR GUARDIAN for every player prior to their first match. Please download, complete, and bring signed form to expedite player check-in. 10 & Under Youth Progression Eligibility Please be aware this tournament has two 10 & Under Youth Progression divisions. ORANGE BALL DIVISION AND GREEN BALL DIVISION. This means that in order to play in a 10 & Under division, you must be eligible for that corresponding level. If you are not able to register for a 10 & Under singles event, that means you are not eligible for that level. To find out more information call PJ Fulmer at (803) 781-2574.

Tournament/Match Check In BRING YOUR MEDICAL FORM TO CHECK-IN Please check-in for the tournament 30 min. early. Tournament check-in will be available beginning Friday at 3:30 p.m. Please check into your site of play. If you are not scheduled to play Friday, the check-in is not mandatory, only check in at least 30 min. prior to your first match.

Tournament Questions Direct all tournament questions to the Tournament Director, Tournament Organizer, or the Referee.

Seeding Criteria The most current USTA Southern Standing List will be used for seeding purposes.

Boys and Girls 10's Green Ball Divisions Boy's and Girl's 10's GREEN BALL divisions will be played on a 78' court using low compression green ball. Draw format will be determined by the number of participants. Scoring format as follows: 2 out of 3 short sets using NO AD scoring, first to 4 games (win by 2). A 7 point tiebreak is played if players reach 4 all. If players split sets then a 7 point tiebreak is played for the 3rd set (win by 2). THERE IS NO COACHING BETWEEN THE 2nd AND 3rd SET BREAK IN THE 10 & UNDER DIVISIONS. Doubles will be a 6 game pro set using NO AD scoring

Withdrawals Withdrawing During The Tournament If you need to withdrawal during the tournament for any reason(medical or otherwise) please inform the tournament director and/or referee. If you are ill and cannot make it to withdrawal in person, please send a detailed email to the tournament director. No-shows will receive suspension points.
Withdrawing Before Entry Deadline:All withdrawals must be made online(not by email) before the
entry deadline. Follow the instructions on the USTA Tennislink page. We are not able to withdrawal you on our end before the deadline.
Withdrawing After Entry Deadline:Once the deadline has passed, the online withdrawal system closes.
Email your withdrawal to the director including your
USTA# and reason for withdrawal. Please note, if the draw has been made it will
be scored as a default. *No refunds will be issued after the online entry deadline.

Important Doubles Instruction If you wish to play doubles and did not sign up with a partner, partners must be declared by Tuesday at 9:00 p.m. via email. Once you find a partner, please email the director your name, your partner's name, and division entering and he will pair you. If you do not find a partner, we may randomly pair you with someone.

Entry Type Entry to this tournament is open to all USTA members. Entries must be submitted by Monday, November 4, 2019 11:59 PM (Eastern Time). Only USTA members can register online if available.

Warm-up Courts: Warm-up courts are available at Shipyard and VDM Tennis Center free of charge at the following times: Friday 2:00 p.m. - 3:00 p.m. and Saturday/Sunday/Monday 7:15 a.m. - 8:00 a.m. Warm-up courts are to be shared with 4 players per court. Private courts are not available during these specified times. If you want to hit outside of these times you will need to call the pro-shop (843-785-8388) to reserve and pay for the court.

Addresses: Van Der Meer Tennis Center 19 DeAllyon Ave. Hilton Head Island, SC 29928 Van Der Meer Shipyard 113 Shipyard Dr. Hilton Head Island, SC 29928 12s and 14s will be played on clay at Shipyard. 10s, 16s, and 18s will be played at hard courts at the Tennis Center. Hard courts at Shipyard may also be used if needed.

World Class Training Practice/training sessions will be available on Friday from 10:00 a.m. - 12:00 p.m. for $25 and/or Friday from 3:00 p.m. - 6:00 p.m. for $35. Please call the sales office (1800-845-6138) in advance to register for these training sessions.
