These supplementary instructions provide information necessary to complete the appeal form:
Introduction
The Self-Rate appeal form is now linked to the Self-rate Questionnaire. Based on your response to the questionnaire, certain fields may be required to be entered and submitted to file a Self-rate Appeal.
It will be possible to submit an explanation when there was an error when completing the questionnaire, and as a result certain fields on the Self-rate Appeal Form should not be required.
Player Contact Info
- NAME, GENDER, USTA Member# and League Team # are pre-populated on the form and cannot be entered or changed (League Team # is filled only when self-rating during League registration).
- Address, City, State, Zip, Home Phone and Email are required. They are pre-filled and can be changed.
- Maiden/Other Name, Work Phone and Fax are optional
- Phone numbers must be entered in (555)555-1234 format
General Information
- Minimum Self-rate assigned is pre-populated on the appeal form and cannot be changed
- Self-Rate requested is pre-filled based on appeal request and can be changed (required)– Select the rating value for your appeal from 2.0, 2.5, 3.0,…… to 7.0
- How long have you played tennis? – total years 0-99
- Rating from another source, foreign or domestic
- If answer is Yes, Rating Level played is required to be entered
- Other Rating Source – 1 can be entered
Sport Affiliations (Enter N/A if they do not apply)
- Affiliation and How long affiliated (years) – Up to 2 affiliations can be entered (e.g., USPTA, PTR)
- Worked as College Coach – Most recent dates
- Worked as Teaching Processional – Most recent dates
TENNIS EXPERIENCE
Junior History (Enter N/A if they do not apply)
- All high school fields are required to be entered, when in question 1: – Highest Tennis Level Played = Played High School tennis within the last fifteen years
- High School Tennis Team – where, State, Year(s) played, Singles or Doubles and whether or not #1 position was played in Singles, Doubles, or both
- Yes response on questions 13, 13B or 15 requires all fields on one line of either Junior Rankings to be completed and/or USTA Tournament info to be completed
- ITF/National/Section Junior rankings - Where, Year(s) played, Type of Ranking and Highest Position or Ranking – Up to 2 entries are available
College History (Enter N/A if they do not apply)
- All college fields are required to be entered on 1 line, when in question 1: – Highest Tennis Level Played = Committed to or played on a varsity college tennis team
- College Tennis Team - where, Year(s) played, Type of Ranking and Highest Position or Ranking, Division
- Select from Division values I, II, III, NAIA and Junior College
- Up to 2 College Rankings can be entered
Professional History (Enter N/A if they do not apply)
- All professional history fields are required to be entered on 1 line, when in question 1: Highest Tennis Level Played = Current or former world class player, or Current or former professional Satellite, Future or Circuit Player
- Select: Foreign, Satellite, Davis Cup, Fed Cup, Pro Circuit, Futures, Challengers, Other from dropdown list
- Enter where, Year(s)played, Type of Ranking and Highest Position or Ranking
- Up to 2 Professional Histories can be entered
Special Sport Recognition/Rankings Achievements (Enter N/A if they do not apply)
- Enter Year and Title for up to 2 recognitions/rankings/achievements during high school and college
Other College Varsity Sports that you lettered in (mark N/A if they do not apply)
- All Other College Varsity Sports that you lettered in fields are required to be entered on 1 line, when in question 16b and 16c: – Did you play a sport other than tennis on a varsity college team is answered Yes
- Use check box to the left of the "Other Sport Name" to indicate that you did play another sport.
- Enter Sport Name, select Level (College), and enter Year(s)
- Up to 2 College Varsity Sports that you lettered in can be entered
USTA League Information (Enter N/A if they do not apply)
- Indicate whether you played in USTA League
- Enter most recent year played in League
- Enter information on the 3 most recent years of USTA League play:

- Select Section from list
- Select USTA League Division Type from list: (Adult, Mixed)
- Select NTRP level from List: (Open, 2.5, 3.0, 3.5, ……..10.0)
- Select Local or Highest Championship reached from list: (Local, Local Playoff, Area/District, State, regional, National)
USTA Tournament Information
- Yes response on questions 13, 13B or 15 requires all fields on one line of either Junior Rankings to be completed and/or USTA Tournament info to be completed.
- Yes response on question 14 requires all fields on USTA Tournament info to be completed
- For Division enter either NTRP and/or Age/Open level
- Enter most recent year played in USTA tournament
- Select divisions played in USTA Tournaments within the last 5 years
- NTRP Levels – (2.5, 3.0. 3.5, …….10.0)
- Age Open – (Open, 16, 18, 25, 30, 35, 40, ……90)
- Multiple divisions can be selected for NTRP Levels and Age/Open
- Enter up to 3 ranking numbers held, year, select division from list (includes NTRP and Age Open values), indicate Singles or Doubles, and District, Section or National

- Describe special conditions, treatment or other information that may affect level of play
- You are required to enter an explanation in the List any other information… box.
