TOURNAMENT SCHEDULING
Girl's 16 & 18s will not start until Sunday. Boy's and Girl's 12; 14 & Boy's 16; 18s will start Saturday. The tournament is a "feed in" format through quarter finals and 3rd/4th playoff for semi losers. Matches will go into Monday and Tuesday. Notes
Five (5) quota spots will earn entry into the USTA/Midwest
Closed Junior Outdoor Championships. The five quota spots will
include the Winner, Finalist, 3rd place, 4th place and FIC
Winner in each division. A wild card will be given to the next
highest qualifier IF one of the top five are unable to register for
the Closed.
Please Note--the endorsement rules have changed: The Play
and Complete Rule will be enforced. A player who does not play and
complete his/her final match in the District Qualifier will
not be eligible for endorsement to the USTA/Midwest Closed.
Play and complete is defined as playing all points including the
final point of the match. A Retirement, Walkover or Default in the
player's final match (including any FIC matches) of the District
Qualifier will cause the player to no longer be eligible to be
endorsed to the Midwest Closed. Quota spots cannot be awarded to a
player that has not won a match at the District Qualifier.
A list of alternates in each division will be submitted to the
USTA/Midwest office. This endorsed list of alternates will be used
for the purposes of filling available spots any district players
withdraw from a quota spot. If a District quota spot is not filled
by the player that earned the spot, it will be filled with a player
from the endorsed alternate pool determined by the USTA/Midwest
Section Standings list. We will not be able to place anyone on the
automatic Quota list submitted to Midwest except the top five
finishers in the Qualifier and one Wild Card if one of the top five
do not plan to go to the Closed. We will submit alternates to be
considered in the overall Midwest alternated pool. Please contact

Proof of Domicile
A player's domicile shall be the domicile of that player's
"parents". Domicile is hereby defined as the bonafide, true and
permanent home of the player's "parents". In the event that the
player's parents are divorced or legally separated, the custodial
parent or the legal custodian who in fact resides with the player,
shall be presumed to be the player's parent for determining
domicile.
In the event of a dispute as to the eligibility of a player under
this section, the following factors shall be used to determine
domicile:
1. A photocopy of the "parents'" most recent state income tax
return or a printed copy of the electronic file form.
2. A photocopy of the "parents" most recent federal income tax
return or a printed copy of the electronic file form.
3. A copy of a Declaration of Domicile, signed by the "parents"
proving that the player's "parents" live in the USTA/Midwest
Section. The document must be notarized.
4. Affidavit of domicile executed by the head of the
household.
5. Photocopies of "parents" and players (when applicable) driver's
licenses and automobile registration (s).
6. Notarized copy of "parents" voter registration.
7. Photocopy of Homestead Tax Credit (all states except Ohio,
unless the "parents" meet the special requirements in Ohio)
The above list is not intended to be definitive or all inclusive
and other factors may be considered by the USTA/Midwest Section
Junior Competition Committee, or may be submitted by the player's
parent, to assist in determining the players' domicile. The
USTA/Midwest Section office staff may request any documents
reasonably believed to be necessary in the determination of a
player's eligibility. The player has the burden
of proof.
A District, player or parent may submit a request for
reconsideration of a domicile ruling to the USTA/Midwest Section
Junior Competition Committee. The decision of the USTA/Midwest
Section Junior Competition Committee or its appointed subcommittee
on the reconsideration request may be appealed to the USTA/Midwest
Section Board of Directors. The decision of the USTA/Midwest
Section Board of Directors shall be final on the issue of domicile.
All requests for reconsideration must be filed with the
USTA/Midwest Section Office at least 30 days, prior to the entry
deadline of any closed tournament in which the player wishes to
enter. Any additional documents requested by the USTA/Midwest
Section Junior Competition Committee shall be submitted within five
(5) business days after their request. The "parents" may submit any
additional documents at anytime prior to the reconsideration review
by the USTA/Midwest Section Junior Competition Committee or its
appointed subcommittee.
The reconsideration shall be considered based upon all documents
submitted to the USTA/Midwest Section Junior Competition Committee.
Any falsification of information will result in a minimum one-year
revocation of junior tennis privileges in the USTA/Midwest Section.
A player suspended for falsification of information may apply for
re-instatement.
The player may apply 30 days prior to the effective suspension one
year anniversary date. Failure to apply for reinstatement will
cause the suspension to continue until such time as
re-application/re-instatement request is approved for said player
by the USTA/Midwest Section Junior Competition Committee or its
appointed subcommittee.
Player Selection TypeNo Selection Process. All registered players will be selected.
Entry TypeEntry to this tournament is open to specified residents only. Entries must be submitted by Tuesday, May 29, 2018 11:59:00 PM Indiana Time. Only USTA members can register online if available.
