USTA MEDICAL RELEASE FORM
The USTA requires a liability and medical release signed by a parent or LEGAL guardian. A player's coach or an adult who is not a LEGAL guardian may NOT sign the form - NO EXCEPTIONS. TOURNAMENT REGISTRATION AND CHECK-IN Friday Night
6:00-7:15pm at Tom Brown Park and Saturday morning 7:00-7:30am at
Tom Brown Park. Bring your completed USTA Medical Release to save
time. You may sign up for doubles during registration. We strongly
encourage you to register on Friday evening. If you wait until
Saturday morning, and if your first match is at FAMU, you will need
to allow for travel time between sites. Late arrival for your match
due to last minute registration will not be excused. GIRLS TAKE
NOTE - ON SATURDAY ALL GIRLS MATCHES WILL BE PLAYED AT FAMU!
PREFERRED METHOD OF COMMUNICATIONS PRIOR TO NATIONAL OPEN
Prior to the event, to facilitate the Tournament Committee's record
keeping procedures please submit communications via e-mail
(will8239@msn.com). During the event, use the tournament desk
numbers, which are shown in the tournament packet.
ALTERNATES TAKE NOTE!! All alternates MUST send me an e-mail
with a telephone number AT WHICH YOU CAN BE REACHED. Some years we
go deep on our alternates lists - don't miss out by failing to
inform us how to reach you! Please, don't just assume you provided
the information when you registered for the event - each year many
applicants either fail to provide any telephone or e-mail contact
information or provide inaccurate information. Also, when you are
admitted to this event you must notify the tournament director at
any site where you are on an alternate list to remove your name
from that list!!
NEXT ALTERNATES - GIRLS (in order) There are no more girl's
alternates. If you know of someone who would like to be an
alternate in the girls division, please have them contact the
tournament director immediately! First come, first served.

MATCH CHECK-IN Check in at the tournament desk at least 20
minutes before each of your scheduled matches.
DOUBLES SIGN-UP ON-SITE SATURDAY Please plan to play
doubles. You MUST sign up for doubles during Tournament Check-in
and Player Site Registration on Friday evening or on Saturday
morning before noon. The first round of doubles will begin Saturday
afternoon. Note: only players may sign up for doubles -- parents
and coaches may NOT sign up players for the doubles event.
WITHDRAWALS Must be submitted in writing and must include
the player's name, USTA number and reason for withdrawal. Refunds
due to illness or injury will not issue unless you comply with the
USTA requirement to submit a letter or other appropriate
documentation from your physician on the doctor's letterhead to the
tournament director prior to the end of the tournament. Address:

checks will issue shortly after the conclusion of the event. Be
sure to provide us with your current mailing address!
HITTING PARTNERS Several college players will be in town
before and during the event. Their rates are very reasonable. Names
and contact information are available upon request. Also, pros and
iinstructors are also available by appointment at USTA
award-winning Forestmeadows Park 850-891-3920 -- ask for Glen Howe
or Steve Leroy.
EVENT SITES Primary sites: Florida A&M University (FAMU)
and Tom Brown Park. Sunday only additional site: Winthrop Park. The
FAMU courts are located on campus, on Wahnish Way off of West
Gaines Street -- watch out for several speed bumps on Wahnish! The
courts do not have a street number, but for Mapquest and GPS units
you can use 1500 Wahnish Way, Tallahassee 32307. Tom Brown Park is
located on Easterwood Drive, which is accessible from either
Capital Circle Southeast or Conner Blvd (closest to the courts).
The courts do have a street number, but for Mapquest you can use
1125 Easterwood Drive, Tallahassee 32311, which is just inside the
park -- from there just follow the winding road to the courts. The
address for Winthrop Park is 1601 Mitchell Avenue - the facility is
on Mitchel between 6th Ave and Betton Road.
PRACTICE COURTS There are several sites in the area that are
available on a first-come-first-served basis. Look for details in
the tournament packet.
HOT WEATHER WARNING Count on very hot weather, and count on
problems if you do not properly hydrate. We suggest that you begin
hydrating several days before the event. We also suggest using sun
screen.
BIG BEND FREEDOM FEST AND FIREWORKS There is a host of
happenings in Tallahassee over the Independence Day weekend,
culminating in an exceptional fireworks show at Tom Brown Park on

NOTES: This is a Level 2 event. Singles is a Full Feed-in
Consolation (through QF), with a playoff for 3rd and 4th place
(awards for 1st - 4th in the main draw, and 5th - 6th in the
consolation round). Doubles is Single Elimination, with a playoff
for 3rd and 4th place. Match Formats: Singles main draw -best of
three tie-break sets. Singles consolation -- two tie-break sets
with regular scoring, in the event of split sets a 10 point
tiebreaker in lieu of full third set. Doubles -- 8 game pro-set. In
the event of inclement weather, formats may change. Balls: Wilson.
New balls are provided at the beginning of a third set. Draw sizes:
64 for singles and 32 for doubles.

Player Selection Type4 players are selected for each site from the most recent National Standings List of the next lower age division and 28 players are selected for each site from the age division of the event
Entry TypeEntry to this tournament is restricted. To see how entrants will be selected, follow the “Selection Process” link to the left. It is the player’s responsibility to check the selection list(s) after registering online to be sure that a check mark appears beside the player’s name on the list, in order to insure proper registration. Entries must be submitted by Thursday, June 3, 2010 12 Noon Eastern Time. Only USTA members can register online if available.
