Medical Release Form
The United States Tennis Association requires a signed medical release covering all entrants in national USTA events. The release must be signed by the entrant and parent or guardian of any entrant who is a minor. The release must be turned into the tournament director at the tournament desk at check in prior to the entrants first match.

On-Site Alternates Registration
On each day, any alternate desiring admittance must check in and be ready for play no later than 7:30, 30 minutes prior to the first scheduled match time of the event in which they are an alternate. The order of alternates is based upon the original Alternate List published by the tournament, not a more recent national Standings List, and not the order in which the players check in.
On site Alternates may be admitted into the singles main draw to replace withdrawing players until the completion of the first round of play. Any player receiving a bye in the first round can also be replaced by an on-site alternate in the second round because this player has not yet played a match in the event. This does not apply to players receiving withdrawals or retirements as these matches are considered won. On-Site Alternates may not play in the Feed-In Championship unless they have played in the singles main draw.
