Medical Release
USTA requires a medical release SIGNED BY THE PARENT OR GUARDIAN for every player. NOTE: A PLAYER'S COACH OR OTHER ADULT WHO IS NOT THE LEGAL GUARDIAN MAY NOT SIGN THE FORM FOR THE PLAYER. The form will be at the end of the confirmation email a player receives when they have been accepted into the tournament. This email should be printed by the parent or guardian, signed and brought to the tournament. PLAYERS ARE NOT ALLOWED ON-COURT WITHOUT THIS SIGNED RELEASE!

WITHDRAWALS
If you need to withdraw before entries close, you must do it online. Click on edit registration, then withdraw. You will get a confirfimation number - KEEP THIS NUMBER! Without the confirmation number you will not have proof that you withdrew. All Withdrawals after entries close must be sent in writing by E-mail to maconten@bellsouth.net to the attention of Donna Bailey. Please be aware that if a player withdraws for any reason other than illness, injury, or a family emergency suspension points can be given to that player. Players withdrawing within 7 days of the tournament start date will not be refunded the entry fee. NO REFUNDS WILL BE GIVEN FOR WITHDRAWALS AFTER SATURDAY 5/9/2009 12 Midnight EST.

DOUBLES
The doubles deadline is Saturday, May 16, 9 AM. If you can't find a partner we will attempt to find one for you. If you didn't enter with a partner, or didn't enter at all, please e-mail us your partner's name, or list your team on the doubles sign-up sheet at tournament check-in. We plan to play at least one, or maybe two, rounds of doubles on Saturday. If you entered online and don't want to play, you must notify us by email. TENTATIVE MATCH SCHEDULE
Tentative Daily Match Schedule
Saturday: 2 rounds main draw singles and one round consolation singles followed by doubles round of 32 and possibly round of 16.
Sunday: 1 round doubles* followed by 1 round of consolation singles, 1 round of main draw singles, a 2nd round of consolation singles,followed by a 2nd round of doubles.
Monday 1 round of consolation singles, 1 round of main draw singles, a 2nd round of consolation singles, followed by the Finals and 3/4 Playoff of doubles.
Tuesday: 1 round of consolation singles, 1 round of main draw singles,followed by a 2nd round of consolation singles.
Wednesday: Singles Final, 3-4 Playoff, Consolation Final.
(* If we have significant rain early in the day, morning doubles will be postponed until after singles on that day.)

PLAYER MEETING AND REGISTRATION
The player meeting will be at John Drew Smith Tennis Center Friday, May 15th @ 6:30 PM EST, All players and parents are encouraged to attend.
All players must register for the tournament prior to playing their first match. Registration will take place only at John Drew Smith Tennis Center. Registration hours are Friday, May 15th from 4 - 6:30 pm EST or Saturday,May 16th from 7am-12 noon EST. After the initial registration, players should check-in at least 30 minutes prior to the scheduled match at the assigned site. When a player checks in, it means that they are ready to play.

SEEDING METHOD
The Computerized List Method of seeding will be used for the tournament. Refer to Table 17 in 2009 Friend at Court for the seeding criteria.

SCORING AND FORMAT
Singles Draw size will be 64 with a Feed-In Championship through the Quarters and 3/4 playoff. Singles Main Draw and Singles Feed in Championship will play the best two out of three tie-break sets. Doubles Draws will be a 32 single elimination with 3/4 playoff. Doubles will play 8 game Pro Set with a 10 point match tie break at 8 games all. On-line doubles registration is preferred however, for the players’ convenience, sign-up for doubles shall remain open until Saturday ( 8 AM ) May 16th, and may be done by email, fax or on site. Doubles play will begin on Saturday, May 16th. In the event of inclement weather, match format is subject to change.

ALTERNATES
It is your responsibility to notify us if you plan to accept a spot in the draw if the opportunity arises. Send us email or cell numbers if you are near the top of the list, please!! If you wish to be removed from the alternate list it must be in writing, please email that request to maconten@bellsouth.net,
The Alternates by Event Report will be updated daily after entries close until the draws are posted. It is the alternates' responsibility to check the website to see if your alternate status has changed. The tournament will make every effort to contact you if your status does change to being in the tournament. Once an alternate is selected for play, payment is due immediately. ONSITE ALTERNATE PROCEDURES
If you are on the alternate list, but have not been admitted by late Friday, May 15, you can be an on-site alternate at John Drew Smith Tennis Center. Alternates will sign in at the tournament desk. If a default or withdrawal creates a vacancy in the draw, the alternate who has signed in and is highest on the alternate list will fill the vacancy. Alternates may play doubles even if they are not admitted to singles.
Effective January 1, 2009, the procedure for accepting on-site alternates into the draw was rewritten for clarification as follows:
The Tournament Committee shall establish a procedure for checking in on-site alternates and instruct players in the Tournament Information Brochure. The procedure shall include the following:
On each day in which alternates may be accepted into a draw, any alternate desiring admittance must check in and be ready for play no later than 15 minutes prior to the first scheduled match time of the event for which they are an alternate.
The order of the alternates is based on the original Alternate List published by the tournament, not a more recent National Standings List, and not in the order in which the players check in.

WITHDRAWALS
Refund of Entry Fees. Effective January 1, 2009, there are new rules related to the refund of entry fees. A player shall now receive an entry fee refund if: • the entry is not accepted • the withdrawal is for previously authorized entry into another tournament or • the Tournament Committee has received written verification no later than the last scheduled day of the tournament that the withdrawal is for injury, illness, or personal emergency. Injury or Illness Written Verification Requirement: If the player is not playing because of an injury or illness, the player shall deliver written verification of the injury or illness from a medical professional ,no later that the last scheduled day of the tournament. Personal Emergency Written Verification Requirement: If the player is not playing because of a personal emergency, the player shall deliver a written deion of the situation no later that the last scheduled day of the tournament and the Referee shall determine whether the situation is a personal emergency. In all other cases, a refund is not required.

Official Ball
Wilson US Open balls are provided by the USTA and are the official ball of this tournament. Players will be provided with three balls for each match, with new balls provided for third sets.

4 players are selected for each site from the most recent National Standings List of the next lower age division and 28 players are selected for each site from the age division of the event

It is the player’s responsibility to check the selection list(s) after registering online to be sure that a check mark appears beside the player’s name on the list, in order to insure proper registration.
