Medical Release/Waiver

Doubles
Doubles sign up will continue through 11 a.m. on Saturday May 17th. Most or all players will play one round of doubles on Saturday night. If you already know who your doubles partner will be, email it to mitchell@southwest.usta.com. Pending selection to the singles draw by both players, we will enter you.

IMPORTANT!!
Tournament directors do not have access to phone numbers and email addresses of the tournament participants UNLESS the player re-enters their contact information at time of event registration. This means that if a player does not enter their phone and email information each time they register for a tournament, the tournament director will not be able contact them for any reason, including withdrawals, defaults, byes, rain delays, etc. It is especially important if you are an alternate. Your cooperation will ensure that tournaments can continue to communicate necessary information to you. Thank you. |

This event is restricted to the following Jr. players: U.S. Citizens; Permanent Resident Aliens; Diplomats, Refugees/Asylees; CTA. Refer to USTA Regulation III.A.2a.

4 players are selected for each site from the most recent National Standings List of the next lower age division and 28 players are selected for each site from the age division of the event
Entry to this tournament is restricted. To see how entrants will be selected, follow the “Selection Process” link to the left. It is the player’s responsibility to check the selection list(s) after registering online to be sure that a check mark appears beside the player’s name on the list, in order to insure proper registration. Entries must be submitted by Thursday, April 24, 2008 12 Noon Eastern Time. Only USTA members can register online if available.
