MEDICAL RELEASE
All players must turn in a medical release/waiver signed by a parent or legal guardian. Please download this form at the USTA web site by clicking on the link provided, so that you will have it at check-in.

LEVEL/FORMAT/SCORING
This is a National Level 2 and a Midwest Level 3 event. Singles draws will be 64 with full feed-in through the quarters and playoffs for 3/4 place. Singles matches will be best of three tie break sets. Doubles will be 8 game pro sets with a 10 point match tiebreak at 8 all. We will be playing two rounds of singles in the main draw as well as one round in the backdraw on Saturday. Doubles will begin with two rounds on Sunday afternoon.

REFUND POLICY
There will be NO REFUNDS of entry fees, for any reason, after the draw has been made (7 days prior to the start of the event)

AIRPORTS
For those of you flying in, the Cincinnati/Northern Kentucky airport is about 40 minutes south of the club and the Dayton airport is about 50 minutes north of the club (quite often,you can get cheaper flights into Dayton)

CHECKIN / STARTING TIMES
Player check-in will be Friday, Feb 15 from 4-7pm at Queen City. First round match times will be available on the tournament webpage on Thursday Feb 14. If you cannot make check-in, you MUST check-in 1 hour ahead of your first scheduled match time on Saturday.

DOUBLES
Players may sign up for doubles at check-in on Friday and the sign up will remain open until noon on Saturday.

PRACTICE COURTS
For practice courts, players may call the club (7 days in advance) to reserve and prepay with a credit card at the rate of $13 per half hour.

This event is restricted to the following Jr. players: U.S. Citizens; Permanent Resident Aliens; Diplomats, Refugees/Asylees; CTA. Refer to USTA Regulation III.A.2a.

4 players are selected for each site from the most recent National Standings List of the next lower age division and 28 players are selected for each site from the age division of the event
Entry to this tournament is restricted. To see how entrants will be selected, follow the “Selection Process” link to the left. It is the player’s responsibility to check the selection list(s) after registering online to be sure that a check mark appears beside the player’s name on the list, in order to insure proper registration. Entries must be submitted by Thursday, January 24, 2008 12 Noon Eastern Time. Only USTA members can register online if available.
