Notes
Category II Championship for USTA Ranking; Level II for
Southwest Section ranking. Trophies will be awarded to winners
& runners-up in each event. Entry limited to 1 singles, 1
doubles & 1 mixed. Tournament directors, in their discretion,
reserve the right to permit payers to enter two singles, two
doubles or two mixed events, where there is a low number of entries
in those events, and where scheduling can be arranged. Maximum of 3
events. Entry fee will not be refunded after the deadline. Please
send in along with check: name, address, email address, phone
#(home, work & cel), birthdate, USTA# & expiration date,
names of doubles & mixed partners, tournament records for last
12 months and rankings(district, sectional or national) if any.
Also include T-shirt size. No fees will be returned after the
deadline. All entrants are invited to attend a cocktail party to be
held at Scottsdale Athletic Club on Thurs Dec. 10, 6:30 - 8PM. A
limited number of accommodations are available at adjacent
Scottsdale Resort Club, 8235 E. Indian Bend Road, Scottsdale,
including: Guest room: $79/night feature a King size bed, a wet bar
with small refrigerator & coffee maker & microwave. Suite:
$99/night: this includes King bed in private bedroom, private patio
or balcony, a separate den with convertible queen sofa, wet bar
microwave, coffee maker, refrigerator 2 27" TVs DVD players &
cable. One bedroom Villa: $149/ night and Two Bedroom villa:
$179/night. Our Luxury villas feature with fully appointed gourmet
kitchen with separate living room and dining area, washer/dryer,
fireplace & private patio, Master bath offers Jacuzzi tub &
walk-in shower. Master bedrooms have a King size bed, there is a
queen size pullout couch in the living room. All Accommodations
