WAIVER
Please sign and bring the attached Consent Form with you to the tournament. You will need a signed form in order to check in. Juniors under 18 will need the form signed by a Parent or Guardian. Check-in will go a lot faster if you bring the form with you.

Entry to this tournament is open to all USTA members. Entries must be submitted by Monday, August 31, 2009 11:59:00 PM Central Time. Only USTA members can register online if available.
