ENTRY FORMS

Go to www.greeleytennis.com and print out entry form or enter from that site electronically and pay with PayPal. Pick up a paper entry entry form at Greeley Recreation Center, 651 10th Avenue, at Family FunPlex, 1501 65th Avenue or at Work Out West, 5701 20th Street. Leave filled entry form with check for entry fee at either one of the above sites. Or mail entry form with check to Alice Smith, 23798 CR 50, La Salle, CO 80645.

ELIGIBILITY

16 years of age or older. If under 19, Directors will place you in a suitable division. This is a non-sanctioned tournament so you do not need to have a USTA membership to enter it.

TOURNAMENT PLAY

Winner/Consolation - teams guaranteed two matches, best 2-out-of-3. Combined rating for Mixed Doubles will be used.

PLAYER NOTIFICATION

Times for the first match will be available after 12 noon on Monday, May 10. Get your time at www.greeleytennis.com by clicking on Doubles Tennis Tournament or call Alice Smith at 970-352-9184. Read the draw carefully.

PRIZES/AWARDS

Winners of each division will receive a custom tournament sweatshirt.
