At the front desk you will be asked to fill out a guest waiver , parents please bring photo ID. If tournament entrant is coming with someone besides parents that are over the age of 18 please email tmorton@lifetimefitness.com to be emailed a sponsoring member form to supply front desk as guests under age are 18 required to come with parents or have a sponsoring member form signed.

Players are selected manually by the tournament director after the close of registration.
