Medical Release Form
All players must have a signed Medical Release form on file with the Tournament Office prior to the start of play - NO Form, NO Play.
Form may be faxed to 210-735-3070; Mailed to SATA, 1503 San Pedro, San Antonio, TX 78212; or emailed to mxdtennis@yahoo.com. PLEASE be sure to add player's cellphone number and Hotel where player will be staying to the top of the form.

On-Site Alternates
Players on the Numbered Alternate list who wish to be an On-Site Alternate should email the Tournament Director at mxdtennis@yahoo.com. We ask that you notify us of your arrival, attend Player Check In and the Mandatory Meeting following check in. On-Site Alternates, if needed, will go in based on the order of the original Numbered Alternate List.

Players are selected from a variety of National Lists and generally require Section endorsement.
Entry to this tournament is restricted. To see how entrants will be selected, follow the “Selection Process” link to the left. It is the player’s responsibility to check the selection list(s) after registering online to be sure that a check mark appears beside the player’s name on the list, in order to insure proper registration. Entries must be submitted by Thursday, July 2, 2015 11:59:00 AM Eastern Time. Only USTA members can register online if available.
