Medical Release Form
Please mail or fax (501-223-0531) a signed medical release form to the Arkansas Tennis Association office, 2024 Arkansas Valley Drive, Suite 302, Little Rock, AR 72212. You will not be allowed on the court without a medical release on file signed by both the player and parent.

Thursday doubles have been scheduled for 2:00 pm. Please keep in mind that some of the matches may be delayed if any the players' singles matches run long. Please feel free to call the tournament desk 501-681-3188 with questions.

Notes
IMPORTANT! Due to a new policy from the USTA's legal department,
tournament directors will no longer have access to phone numbers
and email addresses of the tournament participants unless the
player re-enters their contact information at time of event
registration. This means that if a player does not enter their
phone and email information each time they register for a
tournament, the tournament director cannot contact them for any
reason, including withdrawals, defaults, byes, rain delays, etc.
Your cooperation will ensure that tournaments can continue to

WILL AUTOMATICALLY BE REGISTERED FOR BOTH SINGLES & DOUBLES.
DOUBLES IS NOT MANDATORY. IF YOU DO NOT WISH TO PARTICIPATE IN
DOUBLES, PLEASE CONTACT THE TOURNAMENT DIRECTOR TO BE REMOVED FROM
DOUBLES. DOUBLES PARTNERS MAY BE DESIGNATED AT ANY TIME PRIOR TO

RANDOMLY PAIRED. Tournament Training Sessions: Saturday, August
6th, 2011 Get prepared! And get acclimated to Arkansas - with a
hitting session on the courts you'll be playing on! 2-hour sessions
grouped by ability. Supervised by USPTA Coaches. You may sign up
for more than one session if available. These sessions may sell
out, so get signed up early! Fees are non-refundable after Friday,
July 29th. Each player should check in 15 minutes prior to session
start time. Show up on time! Late arrivals will be replaced with
on-site alternates. To be placed on the alternate wait list, please
arrive 15 minutes before sessions begin. Training Registration:

Court Reservations: Limited courts will be available for
reservation beginning Saturday, August 6th at Burns Park Tennis
Center. Call (501) 791-8585. Tournament Sites: Burns Park Tennis
Center - 4000 Joe Poch Road, North Little Rock, AR 72118 (I-40 to
Exit 150 Military Road) Late Entries Policy: Late entries to the
National Championship will not be considered for the selection
process. Late entries will be accepted by email only. Late entries
will not be accepted by phone. Submit by email your name, USTA #,
birthdate, age division, cell phone # and email address to the
Tournament Director to be placed at the bottom of the alternate
list. Late entry alternates will be placed on the list in the order
that they are received. Alternate Selections: Before the draw is
made, withdrawal of a player that was accepted as part of a
section's quota will be replaced with the next available player on
that section's endorsement list. Withdrawal of a player who was not
accepted as part of a section's quota will be replaced with the
next highest remaining alternate. After the draw is made,
withdrawing players will be replaced with the next highest
remaining alternate. On-Site Alternates: Only those players that
have gone through the registration process and appear on the
alternate list will be considered for on-site alternate
substitution. On-site alternates will be considered in the ranking
order that they appear on the alternate list. Any alternate player
wishing to be an on-site alternate must check in with the desk,
ready to play, 15 minutes prior to the first round commencing.
Alternates checking in after the first round has commenced will be
placed at the bottom of the on-site alternate list, regardless of
ranking. Draw and Match Formats: Singles - 128 Compass Draw - Best
of 3 Sets (4 guaranteed rounds) Singles Consolation - Match
Tiebreak in Lieu of Third Set Doubles - 64 Draw - Single
Elimination - Match Tiebreak in Lieu of Third Set (1 guaranteed
round) Doubles Semi-Finals & Finals - Best of 3 Sets Singles
Draws will be posted on Registration Day - Saturday, August 6th.
Deadlines: Tournament Registration Deadline - Thursday, July 7th,
2011 (12:00 Noon EDT) Doubles Sign-up and Partner Designation
Deadline - 6:30 pm Saturday, August 6, 2011 Withdraw After
Deadline: USTA Regulations with respect to withdrawals and entry
fee refunds at USTA National Championships changed as of January 1,
2009. Immediately email Cindy Curtis if you must withdraw from the
tournament. No refund will be given after the registration deadline
unless the withdrawal is due to an illness or injury AND a written
doctor's verification is provided on or before Friday, July 29th,
2011. Check-in: All players must check-in on Saturday, August 6th
from 3:00 pm to 5:00 pm at the Burns Park Tennis Center. Weather:
